On July 7, 2008, at 0357 local time, a Boeing 747-200FM, N714CK, operated by Kalitta Air as Centurion Airlines flight 164, was destroyed during a forced landing in Madrid, Cundinamarca, Colombia, shortly after taking off from El Dorado International Airport (SKBO), Bogota, Colombia. Of the eight on-duty and off-duty crew members onboard, five were seriously injured and three sustained minor injuries. On-ground injuries included two fatalities and one serious injury. Night visual meteorological conditions prevailed. The international cargo flight, which was destined for Miami International Airport (KMIA), Miami, Florida, was operating under Colombian flight regulations. Under the provisions of International Civil Aviation Organization (ICAO) Annex 13, the United States, as the state of design and manufacture of the airplane and engines as well as the state of the operator and registry, designated an accredited representative who traveled to the scene. Assisting were NTSB airworthiness and human performance specialists as well as technical advisors from the Federal Aviation Administration, Boeing Aircraft Company, Pratt & Whitney and the operator. The investigation is under the jurisdiction of the government of Colombia.
