Factual Information

On March 28, 2008, about 0815, mountain daylight time, a Piper PA-46-350P airplane, Canadian registration C-FKKH, operated by A.D. Williams Holdings Inc., broke up in-flight, and crashed about 12 kilometers northeast of Wainwright, Alberta, Canada. The airplane was reportedly in cruise flight at 27,000 feet msl when the pilot reported a problem with a gyroscopic instrument. The airplane then descended and broke apart. The flight was conducted under the provisions of the Civil Aviation Regulations of Canada. The pilot and the four passengers received fatal injuries. This report is for information purposes only.
