On June 16, 2016, about 1900 Coordinated Universal Time, a Piper PA28R-180 airplane, C-FYGG, struck trees and impacted terrain while attempting a go-around near Green Lake, British Columbia, Canada. The pilot sustained serious injuries and the passenger sustained fatal injuries. The Transportation Safety Board (TSB) of Canada is investigating the accident. As the state of manufacture of the airplane, the National Transportation Safety Board (NTSB) has designated a U.S. accredited representative to assist the TSB in its investigation.
