On August 22, 2018, about 1504 universal coordinated time (UTC), a Piper PA34-220T airplane, Costa Rica registration TI-BDH, impacted terrain under unknown circumstances near Loma Linda, Costa Rica. Under the provisions of ICAO Annex 13, the NTSB has assigned an accredited representative and Piper Aircraft will assist the Costa Rica aviation safety authorities as technical advisor. The investigation is under the jurisdiction of the Dirección General de Aviación Civil (DGAC), Costa Rica.
